  • Job Details
  • Physical Therapist (PT) position at Ascension Health in Racine, Wisconsin.
  • Travel employment type with a contract length of 13 weeks.
  • Requires at least 2 years of acute experience or inpatient.
    • Job Requirements
    • Completed Ascension Specialty Exam prior to first assignment and updated every 3 years thereafter in specialty.
    • Documented proof of two MMR vaccines or Positive IGG Titer required.
    • Fit Test required for all travelers, preferred to be completed prior to start.
    • Documentation of vaccination series completion or declination to vaccine required.
    • Flu vaccine required for the flu season from October 1, 2024, through March 31, 2025.
    • Health Statement or completed physical exam required prior to first assignment, updated annually thereafter.
    • Driver's License/State ID must not be expired and must match address on file.
    • 2 Step PPD OR IGRA (Quantiferon/Tspot) required prior to start.
    • Documented proof of two Varicella vaccines or serological evidence of immunity required.
    • Tdap vaccine must be valid for 10 years from the administered date.
      • Schedule Information
      • Weekends: 1 per month; Holidays: none.
        • Unit Specific Information
        • Must float onsite within skill set or to sites within 40 miles if needed.
          • Additional Details
          • Must show proof of all 12 drugs tested for the Ascension 12 Panel Drug Screen, updated annually.
          • Verification of education relevant to candidate's clinical work must be completed within 90 days of start.
