Job Description:
A company is looking for a full-time Virtual Nurse Practitioner
to join their high-risk home-based care team in Colorado.
Key Responsibilities
Manage patients with complex conditions through tele-assist home
visits and follow-up interventions
Conduct thorough initial and ongoing visits focusing on geriatrics
and chronic condition management
Collaborate with multidisciplinary care teams to ensure aligned
care with patient goals and reduce preventable hospitalizations
Required Qualifications, Training, and Education
2+ years of experience in home-based complex care, geriatrics, or
palliative care as a Registered Nurse or Nurse Practitioner
6+ months of Nurse Practitioner experience
Current licensure in the state of Colorado
Graduation from an accredited NP program with national
certification in Family, Geriatrics, or Adult
Unrestricted driver's license and ability to drive to patient
homes
