Job Description:

A company is looking for a Registered Nurse Case Manager to join their team.
Key Responsibilities

Collects and analyzes performance data to improve patient outcomes and documentation
Ensures safe patient care by adhering to policies and promoting professional growth
Facilitates discharge planning and coordinates care transitions for patients

Required Qualifications

Bachelor of Science in Nursing; Associate Degree candidates must obtain BSN within five years of hire
Minimum of one year of Case Management and/or Clinical Care experience
Must be licensed as a Registered Nurse (RN) in the Commonwealth of Virginia
Basic Life Support certification through the American Heart Association
